"잘 있었어?" is one of the common greetings you can say when you meet your friends who you haven't seen for a long time. In the same situation, you can also say "잘 지냈어?" and this is much natural.
And "잘하고 있어?" can use in many kinds of situations. For example, when your friend is doing her/his papers or essays, you can ask her/him "과제 잘하고 있어?". This means "does it go well?". Here are some more examples:
when your friend is working, you can say "일 잘하고 있어?".
when your friend is going on a diet, you can say "다이어트 잘하고 있어?".
Plus, you can also say "잘돼가?" instead of "잘하고 있어?" and both of them have the same meaning. Here are some examples:
when your friend is doing her/his paper, you can say "과제 잘돼가?"
when your friend is working, you can say "일 잘돼가?".
when your friend is going on a diet, you can say "다이어트 잘돼가?".
